Overview of Cosmetic Breast Surgery

What Is Breast Augmentation?

Breast augmentation is a procedure that uses silicone or saline-filled implants to increase the volume/size of the breasts. The procedure is the ideal choice for women with breasts that feel that their breasts are too small or who would prefer larger and rounder breasts with more projection.

Who is a candidate for breast augmentation?

Just about anyone who is healthy enough for surgery and feels like their breasts are small or that wishes for their breasts to be larger, is able to undergo breast augmentation. Incision Options for Breast Augmentation

There are several different types of incisions that can be used to accomplish breast augmentation. They include:

  • Infra-mammary incision – This incision is placed in the crease under the breast
  • Peri-areolar incision – This incision is placed at the border of the areola (pigmented skin surrounding the nipple) and the breast skin.
  • Axillary incision – This incision is placed in the armpit

Which Incision Does Dr. Mehta Prefer?

“I prefer the infra-mammary incision. The first reason is, I believe this is the “cleanest” incision. When I say “cleanest” I mean this area has the least amount of bacteria. The nipple area has been shown to have a higher level of bacteria because of the ducts that open to the external environment. The armpit is also thought of as having higher levels of bacteria. When placing any type of implant, anywhere in the body, you want to make sure there is as little bacteria as possible to avoid any type of contamination and this is one reason I prefer the infra-mammary incision.

The second reason is, it allows for the best angles and best view for the surgeon to create the “perfect” pocket in which to place the implant with little guess work needed. Any size implant can be placed through this incision whereas there is a limit on the size of implant that can be placed through the armpit incision.

Lastly, the scar hides very nicely in the crease under the breast. It is hard to see. The armpit incision, if visible, will be seen whenever you raise your arms up when wearing anything sleeveless. So, in essence, I am saying more people probably see your armpits more than they do your breasts!”

What Will My Post-Operative Care for a Breast Augmentation Involve?

You will be given all the prescriptions needed during your pre-operative visit. That way you will have plenty of time to have them filled prior to surgery. This operation is not very painful, especially with the pain meds that are prescribed and patients are usually able to move around pretty normally in just a couple of days.

  • No showering until ok’d by Dr. Mehta (usually the next day)
  • Keep arms to your side at all times
  • No driving for at least 1 week or while on pain medication/muscle relaxants
  • No heavy lifting or strenuous activity for 6 weeks
  • No working out for 6 weeks
  • No sexual activity for 6 weeks
  • Wear only surgical bra provided unless directed otherwise by Dr. Mehta

  • Is it better to place the implants over or under the muscle?

    In most cases, Dr. Mehta places the implant under the muscle. There are many advantages to this, mainly having to do with decreased risk of capsular contracture. Also, it gives a more natural look due to the extra coverage the muscle provides over the implant. Placing the implant under the muscles allows for a nicer “slope” to the top of the breasts.

  • How do I know if I need a breast lift?

    This will be determined during your free consultation during which I will examine your breasts and take detailed measurements. I will need to determine your nipple position and assess the quality of your breast tissue. If nipple position is low and/or quality of the breast tissue is not what it needs to be, then a lift will be necessary to achieve the best cosmetic result.

  • Will I be able to breast feed after breast augmentation?

    The way I perform a breast augmentation should not affect your ability to breast-feed. I make my incision in the crease under the breast and place the implants under the muscle. This combination leads to the LEAST amount of damage to the milk ducts of the breast.

  • What about sensation to the nipple?

    Although it is rare, nipple sensation can be affected by breast augmentation. The nipple can become either hypersensitive or can loose some or all sensation. This all depends on how large of an implant is chosen and the location of the nerve that supplies the nipple.

  • How will I have to maintain my breast implants?

    I recommend a maintenance massage that should be done daily for as long as you have the implants in place. This is something I teach my patients once they are healed from breast augmentation. Now, this is not to be confused with the breast massage I teach them to do starting at about 1 week after surgery to help the breasts soften and drop into place. The recommendation with silicone implants is to have an MRI 3 years after having an augmentation and then every 2 years after that. This is to detect rupture of the implant. After breast augmentation, I continue to follow my patients at least yearly in order to assess the breasts for any issues that may arise.

  • How Do I Prepare for a Breast Augmentation?

    Detailed post-operative instructions will be given during your pre-operative evaluation, which usually takes place about 2 weeks before your planned procedure. Here are some general helpful guidelines:

    • Stop all herbal medication, Aspirin products (Alleve, Ibuprofen, Advil etc…), vitamin E and fish oil/Krill oil at least 1 month before surgery as long as it is approved by your physician.
    • Stop ALL tobacco products AT LEAST 6 weeks before your surgery to decrease the risk of healing complications associated with smoking. (This includes patches and gum)
    • Stop all hormone-based medications if approved by your physician at least 1 month before surgery. If stopping birth control please use non-hormonal methods for continued birth control.

    If you are above the age of 40, you will need a mammogram before your surgery dependent upon when your last mammogram was performed.

    Arrange for someone to look after you for at least the first 48-72 hours after surgery. (This can be a dependable and capable family member, friend or a hired caretaker)

    Arrange transportation for your clinic appointments and for your family members because you will not be able to drive for at least 1 week.

    Help will be needed for house work and for small children because heavy lifting or strenuous activity will not be allowed for 4-6 weeks.

    Light foods and plenty of fluids are recommended after surgery with the following as examples: Electrolyte replacement drinks, soups, yogurt with acidophilus/lactobacillus, jello, pudding, crackers etc…

  • Does Dr. Mehta charge patients for follow up visits?

    NO!!! I have an open door policy and do not charge my patients to come and see me after their surgery. If for any reason you feel like you need to be seen sooner than your scheduled appointment you can always call the office and the staff will fit you in.
  • Will my augmentation be covered under insurance?

    No, it won’t. This is considered a cosmetic procedure and cosmetic procedures do not get covered under insurance. Also, Timeless Plastic Surgery is a completely cosmetic based practice so we do not take insurance.
  • What will be provided for me?

    During your pre-operative evaluation (approx 2 weeks before surgery) you will be given prescriptions for a muscle relaxer, pain medication, antibiotic, stool softener and anti-nausea medications.
    Along with other medications during surgery you will get the appropriate dose of IV antibiotics.
    Immediately after surgery you will be provided with and dressed in the appropriate bra needed during your recovery period.
    Immediately after surgery you and your caregiver will be given my personal cell phone number to call on weeknights and weekends with any questions or concerns.
  • How often will I need to be seen for follow up?

    Patient care is of the utmost importance to my staff and I. Therefore, I tend to keep a very close eye on my patients to make sure the recovery goes as smoothly as possible so that any issues can be prevented or taken care of as soon as possible. I will call you the same night of your surgery, once you are at home, to make sure everything is going well and that you and your caretaker have no questions or concerns. The following is a general schedule of appointments but can be changed at any time:

    • Once a week for the 1st 2-3 weeks
    • Once a month for the next 2 months
    • At your 6 month mark from surgery
    • At your 1 year mark from surgery
    • Once a year after your 1 year mark

    There is no charge for follow up appointments. If for any reason you feel like you need to be seen in between appointments or sooner than your scheduled appointment you can always call the office and the girls will fit you in.

  • What if I need to speak to someone after hours?

    Immediately after surgery you and your caregiver will be given my personal cell phone number to call on weeknights and weekends with any questions or concerns.

  • Are there risks for breast augmentation?

    With any surgical procedure there are risks, however, these risks are very rare and one usually is at higher risk of getting into a car accident than these risks. Some of the most common, but still infrequently occurring risks include, but are not limited to:

    • Pain
    • Infection
    • Bleeding
    • Asymmetry
    • Capsular contracture
    • Implant rupture
    • Unhappy with implant size selection
    • Changes in nipple sensation
    • Scarring
    • Fluid collections (seroma)
    • Blood collection (hematoma)
    • Poor wound healing

    As stated before, many precautions are taken by my staff and I to prevent these things from occurring in the first place.
